Second-generation cephalosporin: time above the MIC is the predictive efficacy parameter. Extended Gram-negative spectrum, including beta-lactamase-producing strains of H. influenzae and M. catarrhalis.
Inhibition of cell-wall synthesis by binding to PBPs; axetil (oral prodrug) is hydrolyzed by intestinal mucosa and blood esterases to active cefuroxime.
Oral absorption is enhanced with food (bioavailability from 37% fasting to 52% fed); plasma peaks at 2–3 h; elimination half-life ~1.2 h.
Predominantly renal excretion unchanged; elimination half-life prolongs with severity of renal impairment (~3.4 h in moderate/severe impairment).
Mean elimination half-life ~1.2–1.3 h (125–1000 mg tablets).
