Bisoprolol is a long-acting selective beta-1 blocker with the highest beta-1 selectivity among available beta-blockers. Indicated for hypertension, angina, and heart failure.

Beta-blocker + non-dihydropyridine CCB: risk of severe bradycardia.
Same mechanism as atenolol + verapamil. Both reduce AV conduction and contractility.

Selectively blocks cardiac beta-1 receptors with a beta-1:beta-2 selectivity ratio of 75:1, reducing heart rate and contractility.
Oral bioavailability: >90%. Food does not significantly affect absorption. Tmax: 2-4 hours.
Metabolised ~50% hepatically (CYP3A4) and ~50% renally. No significant genetic polymorphism. Minimal CYP-mediated interactions.
10-12 hours. Elimination: ~50% urine (metabolites and unchanged), ~50% faeces. No adjustment required in hepatic impairment.
